What is Acne and How Prevalent Is It?
Acne is also referred to acne vulgaris. It’s a very common skin condition that can start happening in the teenage years (when hormones start changing) bringing with it shame and depression, including in rare cases suicidal ideation. It develops when dead skin cells, sebum (skin oil), and bacteria plug the pores of the skin and lead to what is commonly referred to as “pimples” or “zits.” These pimples are mostly seen on the face, chest, back, and shoulders and they can range in severity. This condition impacts most teenagers, but moderate-to-severe cases affect about 20% of young people.
Acne can have an incredible toll on your life. It makes perfect sense that people want to treat it with chemicals to get rid of them. But before we discuss the treatment, let’s look at how diet can impact acne.
- Studies show that low glycemic index (GI) diets can lower acne severity. This basically means lowering sugar and carbohydrates and eating more fiber and protein.
- There’s an association between dairy food intake and acne. Cows are commonly treated with hormones to increase milk production and this may be a problem. However, natural hormones also exist in USDA organic cows because they are made to milk constantly even when pregnant, which can change the natural hormones present in the milk. In other words, switching to organic may help, but it doesn’t completely get rid of the problem. Another theory is this is because of whey proteins found inside dairy.
- Natural sunlight or poor hygiene are not associated with the severity of acne, so these are considered myths.
- Studies have linked omega-3 fatty acid and γ-linoleic acid consumption in individuals with acne. In other words, people who eat a diet with more fish and healthy oils would have less acne.
- Studies have also linked probiotic intake to lowering the risk of acne. Here are the brands Mamavation recommends.
Best Skincare Products for Acne May Be Tricky To Spot
We do not want to discourage you from using prescription-strength products, because of the mental health impacts of acne on teenagers. Therefore what we are going to do instead is focus on the other chemicals that are combined with the active ingredients. This will give you a good idea of which brands are aligned with your family when you are making these purchases. In other words, we will be looking at which brands have more “junk” than others present to give you direction. We also encourage anyone who is having issues with acne to consider the dietary impacts we went over above.
We also want to encourage you to try products in the “better” category as well if the ones in the “best” category do not suit you. This is because everyone is different and not all personal care products will work the same on all people. People who are experiencing more moderate-to-severe acne may need to focus more on our “better” category. So feel free to experiment and just use this investigation as a basic guide.
What Chemicals Are Used to Treat Acne
There are many chemicals used to treat acne that are found in Over-the-Counter (OTC) and prescription-strength products. Please note that we are not really focusing on how these chemicals impact your health, but more on how they impact this skin condition. More moderate-to-severe acne may need stronger chemicals to treat.
- Benzoyl peroxide: This chemical is mostly offered over the counter (OTC). Valisure consumer testing found several products containing this ingredient to have concerning levels of benzene. For this reason, we have isolated products with this ingredients in our “not our favorite” section for you to see. In addition, there are some warnings because 4.5% of the population in Japan was allergic to this ingredient and had contact dermatitis after use. Benzoyl peroxide helps inhibit the growth of bacteria Propionibacterium acnes (P. acnes) on the skin, which causes lesions and inflammation. We recommend you avoid this active ingredient completely and we will help you do that.
- Retinoids: Retinoids like retinol, tretinoin, and adapalene can help unclog pores and increase cell turnover. These are all effective treatments for closed comedones, which are also referred to as “whiteheads.” Adapalene is available in 0.1% and 0.3% strength and is the first OTC retinoid used to treat mild to moderate acne and blackheads. It works to regulate the turnover of cells lining the pores. This reduces clogging but can be very drying for the skin. Additional products are by prescription only and this is not considered safe for pregnancy.
- Azelaic Acid: Azelaic Acid is a topical treatment for acne, rosacea, melasma, and hyperpigmentation. Typically it’s a synthetic dicarboxylic acid compound that is derived from a type of yeast that is present on healthy skin. However, it’s not as strong as other alpha hydroxy acids (AHAs) which include glycolic acid and lactic acid, and beta hydroxy acids (BHAs), such as salicylic acid. Because it’s gentler, it has less side effects, but those actual side effects are irritation, dryness, tingling, burning, stinging, redness, or peeling. More serious side effects include blistering or flaking of skin, swelling, hives and itching, tightness of joints, fever, or difficulty breathing. Formulations include both prescription strength and OTC products.
- Alpha hydroxy acids (AHA): This is a water-soluble chemical exfoliant that breaks the bonds that hold dead skin cells together, which in turn can block pores. The two most popular alpha hydroxy acids are glycolic acid and lactic acid. These chemicals have the ability to exfoliate the skin and unclog pores which makes it also very effective against closed comedones. Other types of AHA are citrus acid, malic acid, mandelic acid, & tartaric acid.
- Beta hydroxy acids (BHA): These are oil-soluble chemical exfoliants that enable deeper penetration to effectively treat more severe skin concerns. The most common form of beta hydroxy acids is salicylic acid. Salicylic acid is available over the counter (OTC). It’s naturally occurring or can be synthetically produced and used in anti-acne products. This product can be irritating and thus is restricted in products to low percentages such as 0.5% to 2% strengths. The toxicity of this product is more about ingestion in high amounts, which is not really a problem with anti-acne products. BHAs are best for oily and acne-prone skin. They control sebum production and can clear pores deep down below the skin.
- Niacinamide: This is also known as Vitamin B3, which is essential for skin health. This is preferable to niacin which can cause major irritation. This is found in creams, cleansers, face masks, or serum in 2% and 5% inside formulations.
- Hyaluronic acid: Hyaluronic acid is found naturally inside the body and is responsible for retaining skin moisture and elasticity. It also provides lubrication for the eyes, joints, connective tissue, and other organs. When used as a moisturizer, it draws in and retains moisture to hydrate and protect the outermost layer of skin. When used with amino acids, glycerine, and ceramides it hydrates, smoothes, and protects the skin. When combined with peptides, it protects against free radicals and UV damage while softening the appearance of lines and wrinkles. And when combined with Vitamin C, protects against free radicals, brightens skin, and fades hyperpigmentation. This chemical can also create allergic reactions or skin irritation.
- Sulfur: Sulfur is a naturally occurring element that can treat fungal infections, scabies, psoriasis, eczema, and acne. Typically, it’s combined with other conventional acne medications to battle mild to moderate inflammatory acne. It breaks down and softens the outer layer of skin to dissolve dead skin cells.
Chemicals To Avoid in Anti-Acne Skin Treatment Products
As we were analyzing close to 200 anti-acne products, we found many other ingredients that were more problematic than helpful in these products. Here is a list of ingredients you would find in our “not our favorite” list below.
- Benzoyl Peroxide — this ingredient was recently linked to problematic levels of benzene according to a study via Valisure. “Results from Valisure’s tests show that on-market BPO products can form over 800 times the conditionally restricted FDA concentration limit of 2 parts per million (ppm) for benzene, and the current evidence suggests that this problem applies broadly to BPO products currently on the market. High levels of benzene were not only detected inside BPO products, but also in the air around incubated BPO products, showing that benzene can leak out of some product packages and pose a potential inhalation risk. Incubation of a Proactiv® product at the temperature of a hot car (70°C) resulted in the detection of benzene in a compact car’s volume of air at ~1,270 times the Environmental Protection Agency’s (“EPA”) calculated threshold for increased cancer risk by long-term inhalation exposure to benzene.” Don’t worry, Mamavation has isolated all the products we could find containing this ingredient in the “not our favorite” section for you.
- Undisclosed Fragrance — “Fragrance” is either made from petroleum (synthetic fragrances) or botanical raw materials (natural fragrance) or both. It can also contain ingredients like solvents, stabilizers, UV absorbers, preservatives, phthalates, and dyes. These types of chemicals vary in toxicity. But there is lots of mystery surrounding “fragrance” in personal care products. Companies that make perfume buy chemicals from other companies called “fragrance houses.” This means it is possible for a brand to not even know the fragrance chemicals inside their own product. However, as companies and states are moving toward fragrance ingredient disclosure, this is slowly starting to change. (You can make a difference here by only purchasing from brands that are doing 100% ingredient disclosure to help!) The biggest problem is the mystery because no one has any idea what is inside the word “fragrance.”
- Phthalates: Phthalates are another class of ingredient that is also hormone-disrupting and found in most conventional skin-aging products. Phthalates are commonly used as a solvent (a liquid that can dissolve other substances) and fixative (a substance that can help the fragrance last longer on the skin) inside fragrances. The most common phthalate inside fragrance is diethyl phthalate (DEP) and it’s considered toxic by the Toxic Substances Portal of the Agency for Toxic Substances and Disease Registry (ATSDR). The trade name of this chemical is also known as neantine, peilatinol A, and solvanol, but you are very unlikely to see these names on an ingredient panel. Phthalates are linked to health impacts like obesity, and hyperactivity in children, and the kicker is infertility in men by degrading sperm quality. (In other words, wearing fragrances with phthalates around young boys is a problem.) In fact, phthalates are so problematic to male fertility, they are being restricted in places like China. Phthalates, although worse for men, is also problematic for women.
- Parabens: These are very harsh preservatives used in many personal care products that may lead to hormone disruption because they mimic estrogen. The European Union banned these chemicals in 2012. The most controversial study found traces of five different parabens in the tumors of 19 out of 20 breast cancer patients. Make sure to avoid: methylparaben, propylparaben, butylparaben, ethylparaben, isobutylparaben, & isopropylparaben. Unfortunately, some brands have trace amounts of parabens.
- Formaldehyde Releasers: In personal care products, formaldehyde can be added directly to the product OR it can be released from preservatives inside the product. Chemicals that release formaldehyde over time are referred to as formaldehyde releasers. These chemicals prevent microbes from growing in water-based or aloe vera-based products, but are also linked to cancer and allergic skin reactions. Formaldehyde releasers are chemicals like quaternium-15, DMDM hydantoin, imidazolidinyl urea, diazolidinyl urea, polyoxymethylene urea, sodium hydroxymethylglycinate, 2-bromo-2-nitropropane-1,3-diol (bromopol) and glyoxal.
- Harsh Preservative Combos — Although sodium benzoate and potassium sorbate are safer preservatives used by lots of natural products, when you combine them it’s excessive in personal care products. Our advisors do not recommend this specific mixture of preservatives. It’s okay to have sodium benzoate or potassium sorbate by themselves. We are also approving phenoxyethanol in small quantities for adults, but not for children. Another harsh preservative would be benzyl benzoate.
- BHA (butylated hydroxyanisole ) & BHT (butylated hydroxytoluene): These ingredients are known hormone disruptors that are linked to liver damage. They are another example of harsh preservatives.
- Ethoxylated Ingredients — Ethoxylated ingredients are generally of low concern on their own. However, they can be contaminated with ethylene oxide, a known carcinogen, and 1,4- dioxane, reasonably anticipated to be a human carcinogen. They may also be linked to organ toxicity and reproduction toxicity.
- Polysorbates: Polysorbates are another type of ethoxylated ingredient we wanted to tell you about. They are widely used surfactants in biopharmaceutical formulations and also used as an emulsifier in cosmetic creams, lotions, hair sprays, & skin-aging creams, etc. In other words, it is used to help mix oil and water. Polysorbates are not problematic on their own, but when they used for cosmetic reasons, they are treated with ethylene oxide. Products treated with ethylene oxide frequently have traces of 1,4 dioxane which is a carcinogen.
- Quaternary Ammonium Compounds aka “Quats” — These are chemicals that contain quaternary ammonium compound (QAC) in their chemical makeup and are used in a variety of hair products like conditioners, shampoos, hair spray, mousse, hair spray, and detanglers. They are used as preservatives, surfactants, germicides, and conditioning agents. The Association of Occupational and Environmental Clinics classifies quats as “asthmagens,” meaning they can trigger asthma attacks and cause new cases in asthma-free individuals. Not all “quats” are considered asthmagens, but many release formaldehyde or are linked to hormonal effects. Examples of “Quats” you will find in detanglers are Behentrimonium Chloride, Cetrimonium Chloride, Cetalkonium chloride, Stearalkonium chloride, Cinnamidopropyltrimonium Chloride, Quaternium-18, ditallow dimethyl ammonium chloride (DTDMAC), Olealkonium chloride, & Behentrimonium Methosulfate. Quats that are not concerning inside hair sprays are Guar Hydroxypropyltrimonium Chloride, starch hydroxypropyltrimonium chloride, hydroxypropyltrimonium honey & Cocamidopropyl Hydroxysultaine.
